By self-nominating for consideration for service on a Conference leadership team, you agree to and understand the following:

* You agree that your personal information, including both information collected on this form and information contained in other Conference records (including but not limited to clergy appointment records, leadership assignment history, and others) may be used by the Conference Nominations and Leadership Development Team, including any staff or other Conference leadership who may be assisting that team in its work, for purposes related to the work of that team.

* You understand that this self-nomination will be kept on file for a period of 4 years from the date on which it is submitted, and during that time you may be considered for any vacancies on Conference teams for which you may be well-suited. This consideration will be contingent on your continued eligibility to serve on Conference leadership teams. If at any time you no longer wish to be considered for service on Conference leadership teams, you may withdraw yourself from consideration by emailing nominations@umcna.org.

* If invited to serve on a team, you agree to provide any additional information which may be required to process your nomination in a timely manner, and understand that failure to do so may result in your invitation to serve being withdrawn.

* If nominated and elected to a Conference leadership team, you agree to fully participate in the work of that team for the duration of your term.
